What You’ll Do:

• Provide comprehensive care for patients with chronic kidney disease, hypertension, and electrolyte disorders
• Manage patients on dialysis and coordinate care with transplant teams
• Work with primary care providers and specialists to optimize kidney health and patient outcomes
• Engage in patient education, quality initiatives, and collaborative treatment planning
